Reporting to the Manager of Nutrition and food Services (Birchmount and General sites) of SHN, you will work as an active member of our interdisciplinary team. You will be responsible for providing quality nutritional care and education to patients, their families and/or their significant others within the hospital. In addition, you will be required to contribute to program planning, implementation and evaluation in an interdisciplinary team concept. You will practice according to the current Standards of Practice of the College of Dietitians of Ontario, and the Policies and Procedures of the Nutrition Department and SHN. You may be scheduled at any SHN hospital based on departmental needs.
Responsibilities:
- Providing a specialty-based focus, functioning in a dietitian role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately care for adult clients
- Conducting screenings and client assessments to identify the need for nutritional interventions
- Providing education and counseling for patients and their families, including assisting the client in gaining knowledge, skills and confidence in self-management
- Acting as a resource to members of the inter-professional health care team
- Completing required documentation, including client charting, workload statistics, and quality assurance activities
- Attending clinics, client care rounds and educational sessions
- Participating in the design, implementation and evaluation of continuous quality initiatives
Requirements:
- Bachelor of Science degree in Nutrition, with completion of an accredited dietetic internship program or equivalent
- 1-2 years recent experience as a Registered Dietitian
- Member of the College of Dietitians of Ontario (CDO)
- Demonstrated knowledge of adult learning principles and facilitation skills
- Demonstrated ability to plan programs and develop educational materials to support nutrition education
- Demonstrated exceptional interpersonal, verbal and written communication skills and judgement when relating to clients, families, co-workers and members of the interdisciplinary team
- Demonstrated ability to work effectively within an interprofessional team and independently
- Demonstrated ability to build and maintain effective relationships with a diverse range of people
- Demonstrated ability to organize workload and establish priorities effectively
- Demonstrated problem solving skills and ability to exercise independent judgement and to assess situations and problems efficiently and effectively,
- Motivated self-directed learner, who takes initiative and seeks learning opportunities to enhance client care and professional competence.
- Demonstrated commitment to the philosophy of patient centered care
- Demonstrates a commitment to customer service when dealing with patients, colleagues and volunteers and other members of the community and public
- Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
- Demonstrates an understanding of and commitment to SHN’s comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area.
- Utilize protective equipment provided and identify and reports hazards
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
- Demonstrates excellent work performance and attendance record
Asset:
- Membership with Dietitians of Canada
- Experience working with people from various ethno-cultural backgrounds
- Knowledge of Meditech computer system, Excel and internet
- Ability to speak another language
