Registered Nurse, Diabetes Education Centre
The Diabetes Education Centre (DEC) addresses the needs of diabetes patients. The DEC offers inter-professional collaborative management of care of the paediatric and adult populations with type 1andtype 2 diabetes. Our program also supports women with gestational diabetes. You will have in-depth knowledge of the population with diabetes and use this knowledge to act as a resource to others, including patients, their families/caregivers, and other health care professionals. Your goal in this position is to provide excellent patient and family driven care, in a supportive and culturally sensitive manner. You will do this by collaborating with an interdisciplinary team of care providers, with an emphasis on the assessment, education, prevention, maintenance, and restoration of patient health. You’re a great relationship builder, foster close contacts with peers across the entire organization and have the ability to manage a variety of patient conditions across the life span. You have great interpersonal skills, and effortlessly connect with physicians, staff, patients, and families. You will take on the shared responsibility for ensuring that health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, learners, volunteers and families.

On a practical level, you will
- Provide diabetes self-management education to patients with diabetes and their families/caregivers, on an individual basis as well as in group settings.
- Take a leadership role in developing and participate in centre-specific, as well as corporate patient care initiatives related to diabetes.
- Provide diabetes education to other health care professionals, consult with other health care teams and implement evidence-based practices.
- Participate in the development, implementation, and evaluation of program activities.
- Mentor and teach nursing, medical and other students
- Implement a best practice approach in daily activities.
- Foster an environment that promotes patient/family-driven care and inter-professional collaboration.
Qualifications
- Diploma in Nursing, Baccalaureate Degree in Nursing
- Registered and in good standing with the College of Nurses of Ontario
- Master’s Degree in Nursing, preferred
- Certified Diabetes Educator Certification, required
- Current Basic Cardiac Life Support certification (BCLS)
- Minimum of 3 – 5 years recent clinical experience and expertise in diabetes management and education with a focus on diabetes care preferred
- Insulin Pump experience preferred
- Pediatric, Type 1 Diabetes preferred
- Experience in self-management teaching, adult learning and facilitation
- Experience in assessing learning needs and development and delivery of relevant education programs in a timely fashion.
- Provide education concerning diabetes to patients’ families/caregivers & other health care professionals.
- Bilingualism in French/English an asset
