Job title: Mental Health and Addictions Group Facilitator

Job description: St. Michael’s Hospital Mental Health and Addiction Service (MHAS) provide care to individuals with serious mental illness, addictions and complex medical needs. The primary role of the Group Facilitator is to facilitate and coordinate evidence-based group programming in the acute care (inpatient) and ambulatory settings. There will be a focus on providing Cognitive Behavioral Therapy groups. The role will involve building capacity in group therapies, providing education to students and supervision to other facilitators. Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research/quality improvement and patient and family education are required elements of the position.

DUTIES & RESPONSIBILITIES:

  • Facilitates and coordinates evidence-based group therapy program clients in the mental health and addictions service
  • Coordinates intake and assessment for group therapy program
  • Leads the implementation and evaluation of group programming. Collaborates with the CLM, Medical Director and interprofessional teams to plan the group therapy program
  • Develops and presents proposals for group therapy interventions to meet the needs of patient/families, community and staff
  • Facilitates and monitors a risk reduced, timely and durable discharge and/or care plan process
  • Co-ordinates and facilitates family meetings and case conferences
  • Actively contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community
  • Maintains all required administrative documentation, such as workload statistics, patient rosters, equipment lists, etc.
  • Follows and keeps up to date on hospital and department policies and procedures
  • Is committed to, and participates in, ongoing quality improvement in the organization
  • Promotes professional governance and effective hospital operations through, for example, participation in professional practice councils, unit/departmental meetings and hospital committees

QUALIFICATIONS:

  • Completion of a recognized Master’s Degree in a related Health Discipline
  • Registration as a Regulated Health Professional in Ontario i.e. Ontario College of Social Workers and Social Service Workers (OCSWSSW), College of Occupational Therapists of Ontario (COTO), etc.
  • CPR certification required
  • Minimum 5 years full-time experience working in a clinical capacity in an ambulatory or acute care mental health and addictions settings
  • Demonstrated experience providing psychotherapeutic group programs, including Cognitive Behavioral Therapy and Cognitive Behavioral Therapy for psychosis
  • Experience providing Dialectical Behavioral Therapy and Trauma informed therapy and interventions is an asset
  • Experience providing Motivational Interviewing is an asset
  • Demonstrated experience providing recovery-oriented mental health and addictions care to individuals living with mental illness and/or addictions
  • Previous experience coaching and mentoring students
  • Excellent knowledge of the values, principles and practices of recovery-oriented care
  • Very good knowledge of the social determinants of health as well as the challenges faced by disadvantaged populations
  • Excellent communication and interpersonal skills
  • Excellent oral presentation and writing skills
  • Very good conflict resolution and mediation skills
  • Sound time management and organizational skills
  • Familiarity with quality improvement science
  • Excellent punctuality and attendance record
