What you’ll do
- Coordinate referrals received by the department by methods such as overseeing the referral process: i.e. reviewing, assessing for urgency, prioritizing and making decisions in collaboration with program staff if needed to accept/redirect referrals to appropriate community resources or to alternate internal Eating Disorders or Child/Adolescent Mental Health Program. Maintains appropriate communication with internal or external referral sources as appropriate. Ensure referrals are tracked in the correct database(s) as appropriate. Provide leadership and supervision to staff that may perform elements of this process.
- Coordinate and initiate patient visits such as assessments, consultations, and/or admissions by methods such as gathering information from the referral source/client/other relevant persons, contacting and/or providing pre-care/post care for patient and family, responding to patient, family and health care member’s enquiries, evaluating information to identify priority of service required and suitability of admission into program, liaising with other support diagnostic/health services staff, and organizing team scheduling for assessment, consultation and/or admission.
- Act as a clinical nursing resource by implementation of the Process Manager role for Client Service Plan Management and lead the development and monitoring process of individual Client Service Plans and nursing care plans for Eating Disorders, in collaboration with medical/psychiatric and other interdisciplinary team members from both the hospital and community setting. This includes coordinating and leading Rounds/Client Service Planning Meetings, organizing Team Family Meetings and monitoring treatment review and planning/transition processes for all patients receiving services in Eating Disorders
- Provide clinical nursing support and care for patients undergoing selected procedures including nursing physical assessment as required.
- Review medication requirements in collaboration with the Eating Disorders team. Identify actual/potential complex patient/family situations and collaboratively coordinate care with the interdisciplinary health care team to assist in ensuring effective, high quality patient care.
What you bring
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years recent, related experience with working children, adolescents and families with eating disorders and other Mental Health disorders, or an equivalent combination of education, training and experience.
- Current CPR and CPI training.
You have demonstrated ability to:
- Utilize broad knowledge of BCCNM standards for nursing/psychiatric nursing practice.
- Comprehensive knowledge of eating disorders and broad knowledge of a wide range of other mental health disorders in children/adolescent.
- Comprehensive knowledge and demonstrated skills in individual, family and group therapy.
- Communicate effectively, both orally and in writing.
- Make decisions in an independent manner in a clinical setting.
- Relate effectively to adolescents, children and their families.
- Participate as a member of a multidisciplinary care team, function as a nursing resource team member, and provide effective consultation, leadership and work direction.
