Job description: Your Opportunity:
The Community Care Case Manager functions as a case manager, coordinating and integrating services and care for clients who reside in the community, in lodges or in contracted assisted living sites. Key roles/functions include intake/screening, client engagement, assessment, care implementation, coordination of service delivery, monitoring, evaluating care and outcomes, formal reassessment and disengagement. Case Managers are experienced health professionals who are accountable for facilitating the achievement of client centered outcomes. In order to achieve these client centered outcomes, it is essential that the Case Manager possess strong interpersonal and conflict resolution skills as well as excellent oral and written communication skills. The Case Manager functions as a leader, advocate, educator and steward of resources. Please note, this facility provides Long Term Care (LTC) or Designated Supportive Living (DSL) services and by Order of the Chief Medical Officer of Health, many AHS employees are restricted to work only at a single LTC/DSL site for the remainder of the pandemic. Speak to the hiring manager who will confirm if, as the successful candidate in this position, you will be restricted to work at only one LTC/DLS site in Alberta.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP). Additional Required Qualifications:
Three years recent experience in Community Health setting as a Case Manager (within the last five years) is preferred. Excellent interpersonal, leadership, conflict resolution and communication skills. Excellent time management skills. Excellent critical thinking and decision making skills. Ability to be self-motivated and work independently is required. Ability to monitor financial accountabilities related to care delivery. Physically able to perform all assigned tasks. Basic computer skills required. A valid driver’s license and reliable vehicle are required for the position. Daily Physical Exertion & Tasks can be, but not limited to: Standing, walking, lifting, pulling, pushing, stooping and carrying during the entire shift up to 50 Lbs (~ 23kg). Preferred Qualifications:
