Reporting to the Manager of Women and Program in conjunction with the Manager of Adult Diabetes, the successful candidate will be responsible for assessing, planning implementation and evaluating nursing care and practice activities in a multidisciplinary family centered care environment. The Registered Nurse, DEC will provide inpatient, outpatient and virtual care at the hospital, education, and support to prenatal, paediatric and adult diabetic patients. The nurse will be required to carry the on call phone at times.
Requirements:
- Current certification with the College of Nurses of Ontario
- Certification in Diabetes Education or evidence of working towards the CDE designation
- Competent to perform CPR at a Basic Cardiac Life Support level
- Minimum 3 years nursing experience in a diabetes education center with specific knowledge and experience in both adult and paediatric diabetes, gestational diabetes and intensive insulin management
- Experience in adjusting insulin
- Insulin Pump Experience an asset
- Evidence of sound organizational and prioritization skills with the ability to work well in stressful situations
- Excellent communication skills as they relate to individual counselling, group education, and written documentation
- Interpersonal skills with the ability to work well with patients, families, staff and the community
- Ability to work well independently and to exercise sound judgment
- Ability to perform the essential duties of the position
- Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
- Utilize protective equipment provided and identify and reports hazards
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
- Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
- Demonstrates an understanding of and commitment to SHN’s comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area
- Demonstrates excellent work performance and attendance record
