Responsibilities

Reporting to the Manager of Clinical Outpatient Services and working under the direction of the ICBTT Team Leader, the Case Manager is providing case management services for an intensive community based treatment program for youth and their families experiencing complex disruptive behavior, sexually aggressive behavior and/or severe persistent mental illness.
Main Responsibilities:

  • Assists with the development, implementation, and evaluation of the ICBT program
  • Assists with community based planning
  • Supports and delivers quality patient safety standards within the Northern Zone of Nova Scotia Health Authority
  • Works in accordance with the Occupational Health and Safety Act and PCHA Policies and Procedures and to take every precaution reasonable in the circumstances to protect the employee’s own health and safety and that of other persons at or near the workplace.
  • Serves on various committees as required.
  • Provides verbal and written reports as required or directed by such committees; participates in regularly scheduled meetings relevant to the roles of this position
  • Supports prevention and promotion activities
  • Collaborates with the Child & Youth Team Leader in conducting the assessment processes for ICBT families
  • Develops, implements, and evaluates the treatment programs for ICBT cases such as developing the Individual Family Service Plan (IFSP) for ICBT cases in consultation with the Team Leader and interagency partners
  • Implements and evaluates individual/family treatment plans
  • Reviews and revises care/service plans as part of clinical case review process in consultation with the Team Lead and interagency partners
  • Coordinates case review processes specific to ICBT program including arranging and facilitating case conferences ¬
  • Maintains confidentiality; ensures patients are aware of their rights
  • Assists with processes such as competency, complaints, and protection processes
  • Collaborates with personnel, visitors, and community partners

Qualifications

  • Bachelor’s Degree in Human Services field such as Bachelor of Social Work, Bachelor of Occupational Health or Bachelor of Nursing
  • Registration or eligible for registration in relevant provincial licensing body
  • Equivalent education at bachelor’s level in a non-registerable discipline may be considered with evidence of relevant experience & ability to supervise
  • Demonstrated experience in working with families and youth.
  • Specific knowledge of and experience working with youth displaying disruptive behavior
  • Knowledge of community based multi-system intervention coordination for adults and youth
  • Experience in parent training approaches is required
  • Current non-violent crisis intervention certification
  • Valid driver’s license and/or access to reliable transportation
  • Flexibility in working hours
  • Must possess the ability to make independent decisions within scope of practice
  • Must possess the ability to work effectively with personnel, family members, visitors, agencies, and the general public
  • Must possess willingness to work collaboratively with the Child & Youth Team Leader and participate in ongoing clinical supervision
  • Must have patience and tact as well as be willing to handle patients based on whatever maturity level they are functioning at currently
  • Must be able to work effectively in a crisis setting
  • Must possess an understanding of non-aversive behavioral change and family dynamics
  • Demonstrated ability to cope with and facilitate change
  • Ability to accept and use constructive criticism for personal and professional growth
  • Demonstrated ability to work as a member of a multi-disciplinary team and across agencies
  • Excellent oral and written communication skills required
  • Excellent interpersonal and facilitation skills working with multi level health care provider disciplines.
  • Ability to think creativity, critically and solve complex problems
  • Competencies in other languages an asset, French preferred

Hours of Work

Long Assignment, Part Time (50%) Position; 37.5 hours bi-weekly
Beginning as soon as possible with an anticipated end date of April 11 2022
Dates are approximate and are subject to change

May be required to work overtime on occasion
Travel to various sites within the Northern Zone

Salary Information

$30.1695 – $35.2080 Hourly
