Reporting to the Integrated Stroke Unit Manager, the successful Registered Nurse candidate will be responsible for assessing, planning, implementing and evaluating nursing care and practice activities in a multidisciplinary person centered care environment. You will provide acute and rehabilitative care for patients following an acute stroke.
Requirements:
- Current certification with the College of Nurses of Ontario
- Competent to perform CPR at a Basic Cardiac Life Support level and evidence of continued education
- Demonstrated ability to carry out relevant clinical Best Practices, Knowledge integration, and skills within the management of complex stroke cases that bridge the full continuum of care
- Evidence of sound organizational and prioritization skills with the ability to work well in stressful situations
- Demonstrated experience and proficiency in the provision of effective clinical consultation and care for the complex frail elderly
- Excellent communication, documentation, and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community
- Facilitates application of relevant information to make effective judgments and decisions to lead the management of care.
- Ability to work well independently, exercise sound judgement, proven analytical thinking and proactive problem solving and decision making skills
- Ability to perform the essential duties of the position
- Demonstrated ability to work well collaboratively in a multidisciplinary team
- Utilize protective equipment provided and identify and reports hazards
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
- Excellent work performance and attendance
- Demonstrates values consistent with SHN′s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
- Demonstrates an understanding of and commitment to SHN’s comprehensive safety programs and practices including staff safety, patient safety, and environmental safety and participates in regular in-service education in this area.

- Health Assessment Course or equivalent experience
- Current experience in acute adult medicine and rehabilitation
