Job title: Social Worker, MSW – CP – SJHC SW & Nutrition

Job description: Social workers in health care provide in-depth bio-psychosocial assessments, treatment, best practice based interventions and evaluation for individuals and families. Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education are all part of the social work role.

This role will primarily provide corporate weekend coverage on either Saturdays and/or Sundays and Stat holidays. Weekday backfill for vacation and sick time may be incorporated into the role. The post holder must be willing to work in diverse patient care areas across the hospital including medical, surgical and ambulatory settings.

DUTIES & RESPONSIBILITIES:

  • Able to work effectively as a member of an inter-professional team, collaborating with other disciplines to provide comprehensive patient care;
  • Willing to contribute to the academic mandate of the organization as an educator, researcher, resource or support for staff, students and members of the academic and professional community;
  • Identifies and addresses educational needs for patients and their families;
  • Demonstrates commitment to ongoing professional development in order to continue to provide optimal patient care;
  • Is committed to, and participates in, ongoing quality improvement in the organization;
  • Provides patient care in accordance with all professional and organizational standards;
  • Demonstrates an understanding of the discharge and transition needs of patients and their families
  • Demonstrates understanding of the impact of the social determinants of health and has a strong knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, CAS, Community Housing, CCAC etc.
  • Able to work effectively in an inpatient setting and provide direct patient care based on needs of the patient and priorities of the program.

QUALIFICATIONS:

  • Must have a Master’s Degree in Social Work (MSW) or Equivalent;
  • Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas;
  • Must have experience working in an acute care setting with knowledge and skills necessary for an inpatient and ambulatory setting;
  • Experience working with populations who may experience disadvantage;
  • Experience providing anti oppressive, accessible care for all patients;
  • Current CPR certification;
  • Excellent communication, critical thinking and interpersonal skills;
  • Proven flexibility and adaptability in a fast-paced environment;
  • Excellent time management, organizational and prioritization skills as relates to patient care;
  • Evening work may be required in this role; staff must be available to work weekends.
