Job Summary:

The Addiction and Mental Health Worker will perform duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of participants living in a shared home.

This is a full-time, permanent position that includes enrollment in the Healthcare of Ontario Pension Plan (HOOPP) and extended health care benefits.

Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. In order to avoid delays in beginning a position, interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be delivered and offers of employment cannot be made without receipt of a PVSC.

Job Description and Primary Responsibilities:

Direct Service:

  • Collaborate with participant, participant’s natural supports (e.g. family, significant friends, etc.), and community supports in the development and implementation of an Individual Rehabilitation Plan (IRP), clarifying the roles each plays towards the participant’s rehabilitation
  • Ensure assigned documentation for participants (electronic and paper file) is up-to-date and complete, as per agency standards and within the required time frames
  • Assess and build on participant’s strengths, needs, abilities, and preferences and resources
  • Assess social and environmental factors impacting the participant
  • Motivate and support participant to work towards achievement of their IRP goals
  • Completion of a Mental Health Status Exams within the first week of participant’s admission, and subsequently at a minimum of once per month
  • Medication monitoring and education
  • Support participants with physical, mental health, and addiction concerns, and liaise with external community supports
  • Crisis prevention and intervention to ensure a safe environment for participants and staff
  • Health teaching about mental illness and symptom management
  • Assist and coach participants in the learning and management of activities of daily living, including personal hygiene, daily chores, budgeting, socialization, etc.
  • Support participants with meal preparation; this includes grocery shopping and menu planning
  • Advocate for and support participants in accessing community resources
  • Facilitate and implement participant meetings, small groups and activities
  • Support team by picking up additional responsibilities when needed
  • Promote recreational community outings and activities to enhance participants’ quality of life and provide further connection to the community

Job Specifications:

Vaccination Requirements

CMHA Thames Valley Addiction & Mental Health Services requires that all staff members be fully vaccinated against COVID-19, with the rare exemption of those individuals who cannot be vaccinated due to permitted approved exemptions (medical reasons or religious grounds as recognized in the Ontario Human Rights Code). Proof of full vaccination or approval of exemption, will be required; further details will be provided to those candidates moving forward in the recruitment process.

Required Qualifications

  • Post-secondary education in a health or social service field of study; education as a Personal Support Worker, Development Service Worker or Registered Practical Nurse and/or experience in providing personal care is strongly preferred
  • A minimum of 2 years’ work experience in the addiction and mental health field or residential setting
  • Current Driver’s Licence
  • Access to a vehicle and insurance adequate for transporting individuals
  • Current, clear Police Vulnerable Sector Check (PVSC)
  • Ability to attend work regularly

Preferred Qualifications / Experiences / Qualities:

  • Knowledge of Psychosocial Rehabilitation
  • Residential experience
  • Knowledge of computerized documentation methods
  • Crisis intervention and prevention training and ASIST training
  • First Aid / CPR certification
  • Bilingual in both official languages is an asset

Hours of Work:

  • 70 hours over two week period
  • Rotating schedule of days, afternoons, evenings and weekends
