Position Summary:
- Responsible for development of nutrition care plans and counselling for patients with diabetes.
- Use a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change, i.e. MI, stages of change
- Broad understanding of nutrition resources available in the community to support clients with complex needs
- Functions as part of an inter-disciplinary team and acts as a resource for other health care providers
- Facilitate group education programs at the DMC
- Develops nutrition education resources
- Participates in program planning and evaluation
Key Qualifications:
- Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
- Member in good standing with the College of Dietitians of Ontario
- Current membership in Dietitians of Canada and Diabetes Educator Sector of Diabetes Canada preferred
- Certified Diabetes Educator preferred
- At least 3 years current experience working with clients with diabetes in a health care setting.
- Good working knowledge of diabetes and other comorbidities through the lifecycle and adult teaching principles
- Individual counselling and group counselling experience preferred
- Demonstrated leadership and communication skills
- Positive work and attendance record essential
- Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment
- A second language an asset
